Enrollment Process

Please complete a Child Care Inquiry Form to let us know you are interested in enrolling your child (applies to all ages).​

Every new family must start with this form.

*You may receive this form in person by visiting the CLC office at 905 S 4th St, DeKalb​

If we cannot accommodate your request, your child will be placed on a waiting list for up to six months, unless you contact us to extend the time. New children are enrolled throughout the year according to priority on our waiting list. Many factors affect the time a family may be on our waiting list, including currently enrolled children’s schedules, ages, and development.

If space becomes available to meet your child care needs, we will contact you to make an appointment for Enrollment Orientation. In general, a new child may begin attending CLC two (2) business days after the following requirements have been met.

• Child’s certified birth certificate with the official seal of the issuing county, NOT the footprint keepsake

• Child’s completed physical exam form, either dated within six (6) months of the first day (for ages 6 weeks-5 years) of attendance OR meeting elementary school requirements (for grades K-5)

• Community Coordinated Child Care (4-C) Child Care Subsidy Application (if applicable)

• CLC Enrollment Orientation packet paperwork

• $100 Non-refundable child registration fee

• Deposit equal to one week’s fee or co-payment (minimum $25)

• Classroom Visit with parent/guardian, child, and head teacher (Infant/Toddler, Preschool, and Pre-K ONLY)

No appointment is necessary to visit our center. We are happy to make an appointment if you would like. We recommend that you visit during times when your child’s age group is not napping or on a field trip away from the Center. Please call us at 815-756-3506 for more information.
